Application for a personal licence

Before completing this form please read the guidance notes.

This application forms requires:

two photographs of yourself, one of which is endorsed as a true likeness of yourself by a solicitor or notary, a person of standing in the community or any individual with a professional qualification
any licensing qualification you hold or proof that you are a person of prescribed description
a criminal conviction certificate or a criminal record certificate or the results of a subject access search of the police national computer by the National Identification Service
a submission reference from the completion of personal licence disclosure of convictions form
proof of right to work in the United Kingdom or the share code issued by the Home Office right to work checking service (see note two)
payment of the fee for the application - please have your payment details ready

As mentioned above, in order to begin this application form you must have first completed the personal licence disclosure of convictions form.

At the end of this form you will be given a reference for your submission which you must enter below.
