To make a complaint, you need to complete this form which will be submitted to the council's Monitoring Officer. Please ensure that you include all the information requested. Anonymous complaints will not be considered.
Please note the Committee cannot investigate issues which fall outside the Code of Conduct.
Examples of these include:
- Correspondence - relating to a councillor's failure or delay to reply to written or verbal correspondence
- Non-attendance at public or private meetings
- Behaviour or actions by a councillor outside his/her formal duties
